Medicaid waiver coverage

Medicaid waiver (CLTC Group) data covers all 46 South Carolina counties: Abbeville, Aiken, Allendale, Anderson, Bamberg, Barnwell, Beaufort, Berkeley, Calhoun, Charleston, Cherokee, Chester, Chesterfield, Clarendon, Colleton, Darlington, Dillon, Dorchester, Edgefield, Fairfield, Florence, Georgetown, Greenville, Greenwood, Hampton, Horry, Jasper, Kershaw, Lancaster, Laurens, Lee, Lexington, Marion, Marlboro, McCormick, Newberry, Oconee, Orangeburg, Pickens, Richland, Saluda, Spartanburg, Sumter, Union, Williamsburg, York.

A listing is marked as a Medicaid waiver provider only when a CLTC Group row uniquely matches that license. Counties outside this list are not marked, even if the agency participates.

How a CMS star rating is attached

Star ratings come only from the CMS Care Compare quality-of-patient-care field. A rating is shown when the CMS certification number (CCN) matches one DPH home health license and one CMS row, or when phone, street address, and name produce exactly one pair that agrees on two of those three. If the CCNs disagree, or more than one agency could match, no rating is shown. The directory never fills in a guess.

Data gaps

• 6 of 68 CMS Care Compare rows were not uniquely matched to a DPH home health license. Those licenses show no star rating.
• 1 CCN value pointed at more than one record and was left unmatched.
• 10 of 13 CLTC Group rows were not uniquely matched. The waiver flag is set only on a matched license.
• 19 agencies have no usable phone number in the SC DPH file.
• 1 agency has no county in the SC DPH file.
• 26 licenses list a state other than SC. The state is shown as published.
• 3 CMS matches have no quality-of-patient-care star rating in the CMS file.
• SCDHHS returned CLTC Group rows for non-county buckets (GA within SC Svc Area: 1). They are used only when they uniquely match a license.

Claim a listing

Each agency page links to an email-only claim form. The form does not accept payment. Delivery uses a Cloudflare Pages Function and sends only when the CLAIM_WEBHOOK_URL environment variable is set. A claim does not change the public license record.

Sort order

The default order is alphabetical by agency name. Filters narrow the list. They do not reorder it.
